Your first appointment

Is this your first visit to our practice for treatment? Book an ‘Initial Consultation’ appointment!

You can download and complete the two registration forms (medical history and consent) in advance. On your first visit, you can hand in the forms you have already completed. These will be collected by staff when you register. Alternatively, you can fill in the forms at our reception desk.
